Testosterone Risks and Contraindications - The Case for Coordinating Care

For men over the age of twenty-five, clinical data and scientific advances have led to an increased awareness of the importance of controlling and treating hypogonadism in symptomatic patients. Not only do appropriately treated patients have an improved sense of well-being, they often have improved libido, mood, sexual function, bone density, muscle bulk and strength. Recent studies have shown that a properly managed regimen of hormone therapy may decrease visceral fat, reduce insulin resistance, prevent osteoporosis, and provide cardioprotective benefits. Additionally, treated patients’ level of provider engagement tends to increase as does their willingness to address other aspects of a healthy life, such as proper nutrition and exercise.

Traditional approaches to testosterone replacement therapy have been cumbersome on both patients, and physician-staff. In-office administered IM therapy requires weekly or bi-weekly visits to the treating provider, resulting in a high-utilization rate of the provider’s (often uncompensated) time. Historically, physicians have addressed these challenges by simply issuing a prescription for a "vial" of testosterone, so the patient may self-treat. Unfortunately, unmonitored self-treatment often leads to dosing errors, unchecked adverse reactions or side effects, and less than optimal patient outcomes.

Another treatment option – topical testosterone – is administered at home but is accompanied by high-cost and transmission risk to females and children.

Likewise, pellets require the time and expense of an in-office minor surgical procedure, and carry the additional risks of pellet rejection or infection at the insertion site – outcomes that are not good for the patient, and which generate uncompensated physician interactions due to their occurrence within the global period. And, the inability to address ineffective dosing or mitigate side-effects and adverse reactions often leads to less than optimal results.

TomorrowMed's coordinating care platforms make convenient, low-cost remote treatment of hypogonadism in men a reality, by giving patients access to the most effective and conservative treatment option – intramuscular (IM) injection – while providing physicians the ability to effectively manage and oversee their patients’ care remotely. TomorrowMed's platform allows physicians to provide patients with access to this life changing therapy, by empowering physicians with the tools and technology to manage these patients safely and effectively - with little or no added administrative cost.

TomorrowMed's proprietary platform and technologies connect the patient to the physician through the TomorrowMed app. Among other functionalities, each time the patient administers an injection, the TomorrowMed app notifies the treating physician of the patient's health status, and notable vitals (such as weight, pulse, and blood pressure). If the patient notes any side-effects or issues with treatment, an appropriate in-office encounter can be scheduled. If the physician participates in TomorrowMed's prescription notification program, any expiring prescriptions that may need approval are noted for further physician review. If the patient is enrolled with a TomorrowMed pharmacy provider, TomorrowMed can also notify the pharmacy of a new prescription, or pending refill, so that the patient's medication orders remain uninterrupted.

All patient payments are handled by the patient through the TomorrowMed App, so there is no need for separate billing, or the high administrative expense of collections work from the physician's office.
