How does TomorrowMed work for Physicians?

Eligible PCPs must submit an application, meet certain criteria, complete a background check process, and be accepted by TomorrowMed’s physician acceptance committee. Once accepted into the program, TomorrowMed will coordinate the execution of a Care Coordination Agreement to coordinate care between between the participants involved in treatment.

After becoming a party to the Care Coordination Agreement, a TomorrowMed physician-associate will sponsor your training. This includes providing you and your staff access to TomorrowMed’s proprietary health technology platform and teaching you how to integrate in your clinic environment to enable you to facilitate remote patient care and monitoring and maintain proper record keeping. At your option, TomorrowMed can also connect you to appropriate treatment support faculty. These are collaborating physicians from multiple specialties that are available for discussion on matters ranging from new clinical developments, to management of complex cases. These physicians are available to support you, however you, and only you, exercise medical discretion in treating your patient.

How Does TomorrowMed Work for Patients?

Most patients see a decrease in their overall cost of treatment. Patients appreciate being connected to their doctor, and knowing that their provider is actively managing their care. Because the patient experiences TomorrowMed through a patient-controlled Patient Mobile App, they have the information they need to be proactive in their healthcare at their fingertips.

However, the benefits of care coordination extend further than increased patient participation and compliance. Care coordination involves deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient's care to achieve safer and more effective care. This means that the patient's needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient. Care coordination results in (i) less wait time in physician offices, (ii) a seamless patient experience because the PCP coordinates the care (through the TomorrowMed platform), (iii) the patient does not need to seek treatment outside the PCP's practice, and (iv) the patient's overall satisfaction improves.

There is no up-front or out-of-pocket charge to physicians to participate in Care Coordination. All of TomorrowMed's service fees and expenses are covered within the fee structure of each care delivery model. TomorrowMed is not an insurer, reinsurer, pharmacy, or device distributor. TomorrowMed offers an integrative software platform which facilitates care coordination for physicians offering bundled rates for certain treatments targeted to patients for whom medical necessity has been established through a personal physician-patient relationship.
