Care Coordination is the deliberate organization of patient care activities, to seek to improve the delivery of healthcare services. TomorrowMed's technology is used to facilitate Care Coordination between physicians, pharmacies, and the various constituents involved in a patient's care for common, chronic medical conditions. This Care Coordination allows for the marshaling of resources needed to timely and effectively create a seamless and transparent experience for the patient. TomorrowMed's App allows patients and physicians to facilitate remote patient encounters, receive and process inquiries, exchange information, process prescriptions, coordinate payments, and communicate with the constituents involved in care. Through Care Coordination, patients' needs and preferences are communicated at the right time to the right people, so that goods and services can be timely and effectively delivered, often at substantial cost savings over traditional models of care.

The TomorrowMed platform integrates services provided by the patient's physician, LabCorp, Henry Schein Solutions, Dr. First, various participating pharmacies, payment processors, and TomorrowMed's technology providers to create an integrated approach to coordinated patient care, within specific service lines.
